Personalized Care
Medical Services
Providers function as primary care providers, assessing, diagnosing and treating medical and mental health conditions. Services include: medication management, physical exams (including school and sports physicals), preventative screenings, and sick/problem visits. We offer telehealth and in-office appointments to meet patient needs.
Mental Health
We are a trauma-informed practice that provides collaborative care with psychotherapists and clinical providers. We provide assessment, diagnosis, medication management, evaluation of mental health conditions and various psychotherapy modalities.
